TO USE OUR CHARM EHR APPOINTMENT SCHEDULING, PLEASE SCROLL TO THE BOTTOM OF THIS PAGE AND USE THE "BOOK YOUR APPOINTMENT" INSTRUCTIONS TO REQUEST YOUR APPOINTMENT.  TO BEST USE THIS SELF-SCHEDULE FUNCTION, YOU WILL NEED TO BE ON A DESKTOP, TABLET OR LAPTOP COMPUTER - YOU MAY EXPERIENCE ISSUES IF TRYING TO USE A CELL PHONE.   IF YOU ARE REQUESTING AN APPOINTMENT FOR A TIME <24 HOURS OR MORE THAN 60 DAYS FROM NOW, OR ARE HAVING DIFFICULTY WITH YOUR DEVICE, PLEASE REACH OUT TO THE OFFICE BY PHONE OR PORTAL MESSAGE INSTEAD.   

 

For all new patients, please complete all required portions in the appointment scheduler.  Our office will reach out to you to regarding intake and consent forms that must be completed at least 24 hours prior to your visit. If you have not heard from our office by the day prior to your visit, please reach out to us immediately by phone to assure we have your correct appointment time and contact information.  Please be sure to check your emails and voicemail so that we are able to reach you to confirm your appointment and registration.  WE ASK THAT ALL NEW PATIENTS REVIEW THE FAQ PAGE PRIOR TO SCHEDULING.  WE

For established patients, you may use the link here or the appointment request option in your patient portal to request an appointment. Please be sure you receive confirmation of your appointment, and a Zoom link if you have requested telehealth.  Please reach out to us immediately if you have not received confirmation or your Zoom link.
